The following are the job descriptions of the positions we are opening for application:

Program Director
The program director must possess a minimum of a masters degree in social work and previous experience in working with disabled/elderly adults in a community setting. An equivalent degree and/or experience may be substituted for the educational requirement. The program director should be carefully chosen for his/her ability to assume an administrative, leadership role, and to perform the following responsibilities:

  • direction and supervision of all aspects of the program;
  • supervision of all staff;
  • development and implementation of the program’s outreach plan;
  • coordination of the admission process;
  • performance of program and staff evaluation;
  • fulfillment of the reporting requirements of the Department; and
  • fiscal administration of the Adult Foster Care Program including billing, budget preparation and required financial reports.

The Provider shall arrange for backup coverage for the program director during his/her absence due to illness or vacation.

Registered Nurse
To become one of our home health registered nurses, you must have the basic skills and knowledge in providing a comprehensive assessment of the patient. You have to develop a plan of care under the supervision of the patient’s private physician together with the other members of the healthcare team. You must hold an unencumbered RN license. Home-health care experience is preferred, but we can train you and help you grow your career if you’re able to show a willingness to succeed.

Licensed Practical Nurse
To be a Licensed Practical Nurse, you must have a complete accredited practical nursing program. You must be armed with the general knowledge of medical and nursing care such as vital signs monitoring and catheter inserting. Apart from these roles, you have to ensure the comforts of the patients by assisting them with bathing and dressing, discussing health-related concerns, and reporting the status of care progress to registered nurses and doctors.

Certified Nursing Assistant
A Certified Nursing Assistant helps the patients with their healthcare needs under the demands and instructions of a Registered Nurse (RN) or a Licensed Practical Nurse (LPN). Also referred to as a Nursing Assistant.

Therapist
Therapists are professionals who help the injured and sick improve movement, recover, and manage pain. They can work in a number of fields related to mind and body functioning, helping them deal with mental, emotional, and physical issues. Several types of therapists include respiratory therapists, massage therapists, recreational therapists, physical therapists, family and marriage therapists, occupational therapists, and mental health therapists.

Home Health Aide
A Certified Home Health Aide must show competency in delivering the care to the patients. Our aides work closely and are supervised by our RNs. To become one of our CHHA, you must be eligible to provide services such as medication reminders, light household tasks, personal hygiene care, ambulation, meal preparation, incontinent care, and companionship.

Companion / Personal Care Assistant
Companionship is the kind of service that must be delivered with warmth and tender care. Apart from being a caregiver, a companion care provider should be willing to assist and go with the client’s important appointments.

Homemaker
Homemaking providers are tasked to do the household chores in the client’s shelter. They must ensure to create a safe, secured, and conducive living space for the patient’s recovery and holistic wellness.

Nursing Supervisor
There are a number of responsibilities a Nurse Supervisor have but in general, he/she must be knowledgeable about promoting and restoring day-to-day management and long-term planning of for the patient. Duties may include collaborating with physicians and providing physical and psychological support to patients and families.

Care Manager
As part of the family of healthcare providers, a care manager is responsible in updating patient records and facilitating the overall plan of care, widening the client’s access to other related services available for their convenience and well-being. Their biggest role lies in striving to keep a healthy relationship between the agency and patient. While the assistance is delivered to your home or in any other facilities, care managers are the front line for assessing the patients and their families’ care demands. They lead the team, matching the client’s individual need with the ideal provider and right assistance. Furthermore, they can also get in touch with other insurance companies to know a patient’s plan coverage.

Personal Caregiver
The Provider shall employ or retain services of personal caregivers who are certified personal care homemakers or certified home health aides. Equivalent training or experience could be substituted for the certification. The responsibilities of the personal caregiver shall include executing the approved care plan.

Clerical Staff
The Clerical Staff will ensure adequate clerical support for the agency. The right candidate must have a basic understanding of medical terminology, and have solid experience in office management (typing, billing, communications via telephone, etc).

Community Outreach Representative
Community Outreach Representatives are responsible for the marketing and sales of treatment programs and services, checking the location and its benefits in the industry and target market. They are the ones knowledgeable about Sovereign Health Programs while acting as a resource to external and internal clients/customers. These individuals work in a number of ways that include designing, implementing, and managing referral development and keeping in touch with other outsource healthcare sources. The roles of these representatives are significant to the target clients and their families for they connect the customers with the right access to care. They support adults and individuals with disabilities and their families by looking for reliable sources in the community suited to the care needs and demands.

Clinical Licensed Mental Health Counselor:

  • Assesses patient and family to ascertain emotional and social problems. Utilizes psychosocial assessment tools as a guideline
  • Identifies those issues, which must be addressed during the patient’s care and formulates a plan
  • Identifies the need to involve patient’s family through the initial assessment process and attempts to make contact on the same day, if indicated
  • Consults and collaborates with the other healthcare team members to ensure that patient’s emotional and social needs are addressed
  • Provides psychotherapeutic treatment and/or psychosocial support to patient and family to reduce the impact of identified problems
  • Ensures that inpatient and family remain appraised, involved, supported, and treated through prescribed clinical relationship through at least weekly contacts and whenever a change in care or plan requires more contact
  • I Utilizes licensed mental health counselor technique and theory to assist patient and family in coping with illness or social problem to achieve improved functioning and emotional state

Qualification:

  • Requires Master’s Degree from an accredited school for Mental/Behavioral Health Counselors
  • Requires the level of knowledge of licensed mental health counselor processes and community resources and agencies in relation to health, illness, and disability, generally acquired through one to two years on-the-job training
  • Requires a Massachusetts mental health counselor license or licensed independent clinical social work license
  • Requires advanced interpersonal skills necessary to interact effectively with various healthcare professionals, to develop and maintain effective therapeutic relationships with patients and families, and to supervise graduate students
  • Provides psychotherapeutic treatment and/or psychosocial support to patient and family to reduce the impact of identified problems
  • Requires the analytic abilities necessary to assess patient’s needs, develop associated treatment modalities, and provide effective counseling
